Terms of Agreement

VLA

I hereby affirm that the above information is correct and to the best of my knowledge, complete. This questionnaire fully and accurately describes the problem for which I am requesting legal assistance. I understand that VLA cannot guarantee that it will be able to refer my case to a volunteer or the successful resolution of the legal problem. I agree to promptly notify VLA if I no longer need the assistance of a volunteer. Should my financial situation change to make me ineligible for the services of a legal volunteer, I shall also inform VLA. I acknowledge that I have read and agree to the procedures and eligibility requirements stated in the VLA Policy Manual and/or Referral Guidelines attached to this application. I hereby agree that in the event litigation, arbitration or a similar proceeding becomes necessary, I will not oppose my current Volunteer's decision (or formal motion before a tribunal) to withdraw from representation.

Indemnity Agreement

In contracting with the Arts & Business Council of Miami and Put Something Back I (client) understand(s) that the Arts & Business Council of Miami is a non-profit corporation. For consideration, of which it is acknowledged, client agrees that it will not bring any action, judicial or otherwise, against the Arts & Business Council of Miami and/or Put Something Back, its di-rectors, officers, consultants, affiliates, employees, staff members or volunteers for any injury or damage resulting from any section or omission in connection with the Arts & Business Council of Miami's and Put Something Back's advice, assistance, recommendations, referrals, or other activities. Client also agrees to indemnify the Arts & Business Council of Miami and Put Something Back, its direc-tors, officers, consultants, affiliates, employees, staff members and volunteers from any liabilities attributable to the Arts & Business Council of Miami or Put Something Back, if any, resulting from advice, assistance, recommendations, referrals or other activities in relation to applicant. Client agrees that he/she has carefully read this agreement and fully understands its contents and expressly represents that by agreeing to VLA form, it is understood that this is a contract between client and the Arts & Business Council of Miami and/or Put Something Back, which releases and holds Arts & Business Council of Miami and Put Something Back harmless by client from any liability and that client will indemnify the Arts & Business Council of Miami and Put Something Back from any liability attributable to the Arts & Business Council of Miami and/or Put Something Back
