For small business/inventor groups: Do ALL inventors in your group have a total household income less than 300% of poverty guidelines? All members of the group will attach their most recent IRS 1040 form at the end of the application.

For nonprofit groups: Our budget for past year is less than one million dollars. You will attach your most recent IRS 990 form at the end of the application.

An example of a GOOD description is "the invention involves an improvement to X aspect of Y thing." An example of a BAD description is "by following A, B, C steps, we have achieved X improvement on Y thing.

  1. The information provided by me in this application is, to the best of my knowledge, compete and accurate.
  2. I understand that submitting false or misleading information may result in delay or denial of services.
  3. I understand that misrepresenting the number or identity of inventors, the state of the prior art, or my qualification for micro-entity status to the United States Patent and Trademark Office may constitute fraud, and could result in the invalidation of the patent or application.
  4. I will notify the PATENTS program if any of the information in this application materially changes prior to placement with an attorney, especially if any inventor's income should suddenly increase.
  5. I have not shared any confidential details of my invention with the Patent Pro Bono FL program.
  6. I understand that continuation in the program is at the discretion of the program administrator, and that filling out this application does not entitle me to any services.
  7. I understand it may take up to 120 days to find a matching attorney for my case and VLA cannot guarantee that it will be able to refer my case to a volunteer attorney. If we have not matched your case in 120 days your case will expire and will issue a pro se letter.
  8. I understand that the matter may be placed with a patent attorney, and that this person's representation may be limited to the filing of a patent application for the invention referenced in this application.
  9. I agree and understand that this application does not create an attorney-client relationship between any named inventor or myself and any individual attorney, the Patent Pro Bono FL, Arts & Business Council of Miami or Dade Legal Aid.
  10. I agree not to initiate or pursue any legal action against the Arts & Business Council of Miami, Dade Legal Aid and/or Put Something Back, and their affiliates, contractors, employees, officers, volunteers, and other representatives due to, as a result of, or arising from , any advance, assistance, recommendations, referrals, or other activities in relation to their application or for any injury or damages resulting from any action or omission in connection with the advice, assistance, recommendations, referrals, or other activities of Volunteer Lawyers for the Arts.
  11. I understand I am responsible for paying an administrative fee prior to placement with a volunteer patent agent or attorney, and I understand that the applicant remains responsible for the payment of all necessary governmental fees and ancillary fees, e.g. draftsmen, when necessary.
